

An Edgewater pedestrian accident involving a single vehicle claimed the life of a man after he was struck while attempting to cross the road. The incident took place at approximately 12:00 AM on Friday, May 4, 2012, in the 300 block of North Ridgewood Avenue in Volusia County.
According to reports, 68-year-old John L. Gibson was traveling westbound on foot in the west lane of North Ridgewood when he was struck by a car. The force of the impact threw Mr. Gibson nearly 70 feet. Unfortunately, Mr. Gibson lost his life due to injuries sustained in the collision.
The driver of the vehicle that struck Mr. Gibson has been identified as Seth H. MacPherson, 40, of New Smyrna Beach. Mr. MacPherson has a history of driving citations that includes careless driving and speeding. In addition, records indicate that he completed drug offender probation last summer and spent 10 months in prison for possession of methamphetamines.
The fatal Edgewater pedestrian accident remains under investigation by local authorities. There was no immediate indication as to why Mr. MacPherson was unable to avoid striking Mr. Gibson. Toxicology results are pending.

A car accident in The Villages involving a single vehicle claimed the life of one and left another facing DUI and manslaughter charges. The incident took place at approximately 2:30 AM on Sunday, April 29, 2012, along Buena Vista Road in Lake County.
According to Florida Highway Patrol (FHP) officials, the driver of a 1999 Saab was traveling Northbound along Buena Vista Road when he lost control of his vehicle and slid sideways into a lamp post. The driver has been identified as Charles J. Kusiak, 23, of The Villages.
Unfortunately, Mr. Kusiak’s passenger lost his life in the collision. Paramedics pronounced Robert J. Kokinakis, 22, also of The Villages, dead at the scene of the accident.
According to Cpl. D.A. De Weese, Mr. Kusiak lost control of his vehicle while negotiating a left curve. The car slid into the lamp post and then backwards into a tree after rotating counter-clockwise.
Rescue crews transported Mr. Kusiak to Leesburg Regional Medical Center where he was treated and released for minor injuries. He was then transported to the Lake County jail where he faces DUI and manslaughter charges.
The fatal Villages car accident remains under investigation by local authorities. Investigators indicate that speed may have also played a role in the crash.

A Volusia County car accident involving two vehicles, including a school bus carrying special needs students, left two injured after the bus was hit from behind. The incident took place at approximately 7:45 AM on Thursday, April 26, 2012, at the intersection of Interstate 95 and LPGA Boulevard.
According to reports, the school bus was stopped at a red light at the intersection of I-95 and LPGA Boulevard when it was hit from behind by a car. The bus was transporting 26 special needs students and two employees from the Volusia County School Board to Champion Elementary School when the incident occurred.
Unfortunately, two people were injured as a result of the collision. Rescue crews transported one of the assistants and a child to a local hospital where they were treated for injuries resulting from the wreck. Their injuries have been described as minor, and they were later released from the hospital.
The two-vehicle Volusia County car accident remains under investigation by local authorities as they try to determine the cause of the accident. The driver of the car was cited for careless driving.

A Palm Coast car accident involving a single vehicle left 10 people injured after a woman crashed her car into a local store. The incident took place on Saturday, April 14, 2012, at the Publix in Palm Coast.
According to reports, a woman lost control of her vehicle and plowed through the entrance of the Flagler County grocery store. She has been identified as Thelma Wagenhoffer, 76, of Palm Coast. She was not injured in the wreck.
The Florida Highway Patrol reports that 10 others were injured as a result of the crash. Two of the victims, including an elderly man who was left pinned beneath the car, suffered serious injuries. The other victims include a baby and a teenage girl. Fortunately, their injuries were minor.
Ms. Wagenhoffer said that she didn’t know what caused her to crash into the store.
“She’s trying to put the pieces together,” said Alexander Wagenhoffer.
Flagler County Sheriff’s Office spokesperson Debra Johnson said that Ms. Wagenhoffer pulled up to a stop sign where she may have accidently stepped on the gas instead of the brakes. Police are also trying to determine if Ms. Wagenhoffer’s car experienced any mechanical failure which may have contributed to the accident.
The single-vehicle Palm Coast car accident remains under investigation by authorities.

An Orange County car accident involving seven vehicles, including a special-needs van, sends 11 people to the hospital. The incident took place on Wednesday afternoon, March 28, 2012, along W. Colonial Drive.
According to the Florida Highway Patrol, the driver of a special-needs van lost control of his vehicle as he was approaching the intersection on W. Colonial Drive and Paul Street. In an attempt to avoid the vehicles in front of him, which were stopped at a red light, the van swerved into the outside lane and flipped several times before striking the other vehicles.
Rescue crews transported 11 people, including six from the special-needs van, to local hospitals where they were treated for injuries sustained in the crash. Information concerning their injuries and identities was not immediately made known.
The driver of the van has been identified as Travis Richardson, 20, of Orlando. He was cited for careless driving. It was unclear as to what company he was driving for when the accident took place.

An Orlando car accident involving three vehicles leaves five injured, including 3 children. The incident took place at approximately 10:45 AM on Friday, April 6, 2012, near State Road 408 in Orange County.
According to reports, three vehicles collided along Dean Road near State Road 408. There was no immediate indication as to what caused the three vehicles to crash.
Rescue crews transported three children to Arnold Palmer Hospital in Orlando where they were treated for injuries sustained in the Orange County crash. One adult was taken to Orlando Regional Medical Center while the fifth victim was taken to Florida Hospital East for treatment. The identities and conditions of the five victims were not immediately made known. There were no other injuries reported.
Dean Road was closed for an undetermined amount of time while crews cleared the accident scene and authorities conducted their investigation. It was eventually reopened.

A Flagler Beach car accident involving a T-bone collision left three people injured after the driver of a Ford Mustang lost control of his vehicle and crossed into oncoming traffic. The incident took place at approximately 3:00 PM on Saturday, March 31, 2012, along State Road 100.
According to reports, the driver of a 2006 Ford Mustang lost control of his vehicle in the rain near Jarosz Street and slid across the grass median where he was T-boned by a westbound pickup truck. The driver of the Mustang, Nathaniel Columbie, 22, of Palm Coast, was taken to Halifax Medical Center in Daytona Beach where he was treated for critical injuries.
Rescue crews transported Mr. Columbie’s passenger, Jessica Laliberte, 21, of Palatka, to the hospital where she was treated for serious injuries. The driver of the pick-up truck, Preston Hagens, 18, also of Palm Coast, was taken to Florida Hospital Flagler. He suffered serious injuries as well.

A Deltona bicycle accident involving a car leaves a teen injured after he was struck while crossing the street. The incident took place at approximately 7:15 AM on Wednesday, April 3, 2012, at the intersection of Howland Boulevard and Day Road in Volusia County.
According to the Volusia County Sheriff’s Office, the male teen was headed to school when the incident occurred. The vehicle involved in the incident was stopped at a stop sign when the student began to cross the street in front of her. The bicyclist was under the impression that the female driver saw him. Unfortunately, she did not and hit him while she was attempting to turn.
Rescue workers treated the 18-year-old senior from Pine Ridge High School at the scene of the accident. He was treated for a bruised wrist, but was not taken to the hospital.
Since August 20, 2011, there have been 20 other students hit by cars on their way to or from school in Volusia County. The most notable incident claimed the life of a 16-year-old boy in Orange City.
The Deltona bicycle accident is still under investigation by local authorities. It was unclear as to whom was at fault or if any charges were expected to be filed.

A Port Orange pedestrian accident involving a hit-and-run driver sent a man in a wheelchair to the hospital after he was struck while trying to cross the street. The incident took place on Sunday afternoon, March 18, 2012, along Ridgewood Avenue in Volusia County.
According to reports, a man was attempting to cross Ridgewood Avenue in a motorized wheelchair when his wheelchair was hit by a southbound pickup truck. The force of the impact sent the victim onto the pavement where he was hit by another southbound vehicle, driven by 26-year-old Brittany A. Ellis.
The victim has been identified as Donald L. Seymour, 74, of Port Orange. Rescue crews transported Mr. Seymour to Halifax Health Medical Center where he was listed in critical, but stable condition.
The driver of the pick-up truck fled the accident scene. However, he was picked-up in New Smyrna Beach. The hit-and-run driver has been identified as Ricardo P. Oliveira, 50, of Ormond Beach.
The Port Orange pedestrian accident remains under investigation by local authorities. Mr. Oliveira was charged with leaving the scene of an accident with personal injury. A Daytona Beach pedestrian accident involving a single vehicle claimed the life of a man after he was struck while walking along the road. The incident took place at approximately 7:30 PM on Thursday, March 1, 2012, along Nova Road.
According to local police, a man was walking in the 800 block of Nova Road when he was hit by a car traveling southbound along Nova Road from Brentwood Avenue. The pedestrian has been identified as 61-year-old Ralph Albert.
Rescue crews transported the victim to Halifax Health Medical Center where he was treated for extensive injuries. Unfortunately, he was later pronounced dead from injuries sustained in the accident.
One witness reported seeing Mr. Albert staggering along the west side of the road moments before the accident occurred. He was approximately 100 feet from the intersection when he was hit.
The driver of the car has been identified as Sandra Dangelo of Daytona Beach. She was not injured in the crash.
The fatal Daytona Beach pedestrian accident is still under investigation by local authorities. Police spokesperson Jimmie Flynt indicated that no charges are expected to be filed against Ms. Dangelo.
