What You Will Need

In order to apply online for disability benefits through SSA, you’re required to provide:

  • Background information
  • Disability information
  • A signed medical release
  • Confirmation of the required information

The online form guides you in the correct order and prompts you to input the necessary information. If you do choose to complete the form in one sitting, record your re-entry number, which will be provided to you upon completion of the background portion of your application. This number allows you to return to complete the form at a later time.

What If I Don’t Have All of the Information?

You may toggle from tab-to-tab at the top of the form page and skip sections for which you do not have all of the required information. However, it’s important to save what you’ve entered so far before changing tabs; otherwise any previous entries are lost.

Upon completion of the online form, you’ll receive:
